Position Summary:Evaluate each consumers need for nursing care on an on-going basis and provide such care and Residential programs.

Essential Accountabilities

+ Coordinates medical pre-admission evaluate and screening of prospective consumers.

+ Reviews and update consumers pre-admission evaluation within thirty (30) days of placement.

+ Develops the annual nursing care plan.

+ Reviews and revise the annual nursing care plan at least quarterly.

+ Transcribes medication orders and dispenses daily medication to consumers as indicated.

+ Reviews monthly - the medication record of each consumer for potential adverse reactions, allergies, interactions, contraindications, rationality and laboratory test modifications. Advises the physician of any medication errors and indicate changes.

+ Reviews special dietary orders and ensures accurate documentation of blood pressure and weight for consumers.

+ Provides first aid and daily nursing care to consumers as needed. Monitors consumers medical condition through observation and examination

+ Provide on-going care and treatment of medical issues as prescribed by the Nursing Care Plans and/or physicians instructions, including but not limited to, providing soaks, changing dressings and managing G tubes and colostomy bags etc.

+ Escorts/accompanies consumers on medical or psychiatric appointments as needed. Participates in the weekly consult review meetings and ensures that all appointments/evaluations are completed in a timely manner.

+ Serves as assigned on emergency medical response team. (Day Program settings)

+ Provides on-going staff training in

+ Detecting signs and symptoms of illness

+ Specific health needs of the consumer

+ First-aid

+ Proper oral hygiene methods

+ Nutrition

+ Infection control, including universal precautions and OSHA standards.

+ Hospitalization and emergency medical procedures

+ Conducts AMAP training courses as required by the Health Care Coordinator or conducts initial and annual medication pouring observations for all assigned staff.

+ Participates in the control of communicable disease and infections through the infection control committee and implementation of appropriate protective and preventive measures.

+ Administer Hepatitis Vaccine and PPD test to staff as scheduled

+ Acts as a liaison with physician, dietitians, hospitals, pharmacies and families for all medical situations.

+ Participates in Interdisciplinary Team Meetings.

+ Serves on agency and department committees as requested.

+ Maintain confidentiality at all times.

+ Other duties as requested.


Must have a current NYS RN License. Minimum one year of experience. Experience treating the developmentally disabled preferred.
